Every entry carries where it came from. A page, a person, a phone call with a date, or honestly just you working it out, which is a perfectly good answer.
It only ever answers from what you recorded, and says which entry it came from. It never fills a gap from its own knowledge and files it as though you found it.
It never turns into advice. Recording what you were told is fine. Telling you what that means for you is not, and that line holds hardest around money, tax, law and health.
Only the few things that genuinely expire get a review date. A library that keeps asking to be re-checked is one nobody opens.
