It keeps records. It does not give advice. It will not tell you whether something is safe, what a symptom means, or what a combination does, and it will not suggest starting, stopping or changing anything. When you ask anyway, it writes the question down so it actually gets asked at your next appointment.
Nothing is ever filled in from what Claude thinks it knows about a medicine. Everything comes off your own packet, label or letter, or out of your own mouth.
If you keep a day to day record, it records what you tell it and never draws a conclusion from it. It can show you your own entries, which is exactly what makes an appointment useful.
All of it lives in your own Notion, on your account. It does not come to us, we never see it, and this page only ever receives the four answers you give below, none of which ask anything about your health.
